PATIENT NOTICE

On October 15, 2009, a theft occurred at Highland Community Hospital. Radiology films, that had been scanned into the hospital's computer system and were scheduled to be destroyed, were taken from the facility. Less than 300 films were taken and they ranged in dates from February 17, 1997 to May 11, 2008. The films contained patient names and dates of birth, a reference number, and type of radiology image, but did not contain Social Security Numbers, credit card information or any other financial data. Highland Community Hospital immediately reported the theft to local law enforcement and is making credit monitoring available to those patients whose films were stolen.

Highland Community Hospital places high priority on the privacy and confidentiality of its patients' medical records. Be assured every available step to protect the security, confidentiality and integrity of all information relating to its patients has been taken.
